The reason we measure carefully first is clearance: a mantel shelf has a minimum distance above the opening, and it increases the further the shelf projects. We'll tell you before fabrication if the design you want can't be built to those clearances, because the alternative is finding out at installation with a piece of stone that can't be returned.
